Physician enquiry privacy notice

Scope: This notice relates to the professional-enquiry form on the JivaAyush Clinical Collaboration website. This notice describes the current email-based enquiry flow on this website and should be updated if a stored form or CRM backend is later connected.

What you provide

Your name, speciality, medical registration number, clinic/hospital, city, email, optional phone, enquiry topic and any professional-only message you choose to submit. Do not provide patient identities, medical records or any health information in this form.

Purpose

JivaAyush uses this information to verify and respond to professional enquiries, share requested product and scientific information, and follow up concerning the collaboration you requested. You may request that professional follow-up stop at any time.

How submissions are handled

The website does not store enquiry submissions in WordPress. Selecting the enquiry button prepares an email addressed to sales@jivaayush.com in the visitor’s email client; the visitor decides whether to send it. If sent, handling follows the email account and JivaAyush’s internal access, security and retention procedures. This notice must be updated before a stored form or CRM backend is enabled.

Contact sales@jivaayush.com to ask about an enquiry, request correction, or ask that your information be deleted, subject to applicable legal obligations. Do not email patient records to this address.
